These forms accompany the blog post "Improve your ODK forms in 6 easy steps". There are 2 files here: the first, "Caffeine V1", is an example of a basic and poorly designed XLS form, and the second, "Caffeine V2", is an improved version of the same form. Both forms include many comments within the Excel files themselves.Also included are 2 links to online guides; both are useful for writing ODK forms. xlsform.org is an excellent resource for learning the core features of ODK. The ODK documentation link is recommended for more advanced users who want to find answers to specific questions.
